For a hypothetical referral, keep three items together: the first clinic’s consultation record, the specialist’s itemized estimate and the insurer’s instructions for related invoices. A bill for a diagnostic consultation may arrive before a treatment plan is known. Ask which service fees are included in the offered insurance benefits; a future procedure’s possible eligibility does not by itself settle the earlier examination fee.
Do not combine different practices’ charges into a rewritten invoice. Keep the originals and label them by provider and service date. That makes it easier to explain the sequence without changing evidence.
